Temporary Financial Assistance Available

To be eligible for this program, you must meet the following critiria:

• Have a child with a physical or mental disabilty diagnosed within the last 6 months (the diagnosis can be prenatal or at any age)
• Be at or below the State of New Mexico's poverty line

To apply, please do the following:

1. Pick up an application from ChildLink (address below)
2. Assemble the following papers:
   1. Your completed application
   2. Doctor's records for your child's diagnosis
   3. Tax records or New Mexico Social Services certificate of your income level
   4. Your lease, mortgage, or medical bills, depending on which you need help with
3. Make an appointment with a ChildLink volunteer, available these items:
   1. Ida:MW 10:30 a.m. to 3:30 p.m.
   2. Juan:TR 9:00 a.m. to noon
   3. Chris:F 10:30 a.m. to 3:30 p.m.
